Fees for Dr. Hayman
New Patients - 15 Minute Initial Consultation - No Charge
This initial phone consultation gives you the opportunity to see if Dr. Hayman’s style of medicine is the best fit for you.
Initial Comprehensive Visit - $350
During your first visit, the Dr. Hayman will spend one to two hours to review and discuss your chief concerns and medical history. Typically, she will gather lab work and research your case after your initial visit. This will allow her to give you the most comprehensive care. Follow-up appointments are scheduled one to three weeks after the initial appointment. The cost of the initial visit depends upon the time and complexity of the visit. Most new patient visits are 1.5 hours.
New Patient Intake: $350
$100.00 for additional 30 minutes to new patient consult
Dr. Hayman provides acute care to established patients only.
Continuing Patients Follow-up Visit - $100- 200
After the Initial Comprehensive Visit, Dr. Hayman will schedule an in-office follow-up appointment to check-in on the progress of a treatment and to go over any lab work or imaging ordered from the initial visit. Depending on the case, this can also be done over the phone and/or Zoom if the patient does not live locally. Typically, a follow-up visit after the initial visit costs between $100- 200. The fee is based upon the time and complexity of the follow-up.

$200 for 46-60 minutes
$250 for 61-90 minutes
Phone Consults - $45 - $200
Established patients have the ability to do Skype or phone consults with the doctors. These are a great resource to established out-of-town patients. Any time period greater than five minutes is considered a phone consult. The cost for the appointment includes the phone call as well as doctor’s time spent before and after the call (office time such as reviewing labs, editing the treatment plan, calling in prescriptions, etc. which would otherwise be done while the patient is in office). The cost of phone consults varies by time spent:
$100 for 5-30 minutes
$150 for 31-45 minutes
$200 for 46-60 minutes
$250 for 60-75 minutes
$300 for 75-90 minutes
Email Correspondence - $35 - and up
We strive to make our care as convenient as possible for our patients. However, any email that requires a complex answer or takes more than five minutes of the Doctor’s time may carry a fee similar to a phone consult.
Refill Requests - $35
Any refill request outside of a regularly scheduled office visit may carry additional fees.
Cancellation Policy:
A deposit of $50 is required for all new patient appointments. Please let us know as soon as possible prior to your appointment if you can’t make it or would like to reschedule. Cancellations with less than 24 hours notice, or a no call no show will result in forfeiture of your deposit.
